Onychomycosis
Fungal infection of the nails is a common but difficult to treat issue. It can cause nail discolouration, thickening and sometimes pain. Topical creams and paints are mostly ineffective. Laser treatment through the nail has a good clearance rate.
Causes
Onychomycosis is caused by fungus getting under the nails. This can occur more frequently if the nails or cuticles are cut / manipulated in a pedicure, damaged by marathons / walks or poor fitting shoes, or if you have tinea skin infections or frequently attend pools / communal showers / travel.
Symptoms
Tracts or linear discolourations can appear under the nail with white, yellow or brown changes. Nails can become thick or brittle. Sometimes there is pain.
Treatments
Topical creams / paints are about 2% effective.
Laser treatment at GVSL with 3 x monthly sessions of 1064nm nail laser provides a clearance rate of 70%, completing more sessions increases the effectiveness.
Oral antifungal tablets taken for 3 months are 65% effective.

The laser feels like a warm sensation on the toes that suddenly builds up to an intense heat like touching an oven, this is the endpoint that we stop, so a very brief intense heat is the only discomfort.
